It is a graph of voltage versus time of the electrical activity of the heart [4] using electrodes placed on the skin. These electrodes detect the small electrical changes that are a consequence of cardiac muscle depolarization followed by repolarization during each cardiac cycle heartbeat. Changes in the normal ECG pattern occur in numerous cardiac abnormalities, including cardiac rhythm disturbances such as atrial fibrillation and ventricular tachycardia , inadequate coronary artery blood flow such as myocardial ischemia and myocardial infarction , and electrolyte disturbances such as hypokalemia and hyperkalemia. In a conventional lead ECG, ten electrodes are placed on the patient's limbs and on the surface of the chest. The overall magnitude of the heart's electrical potential is then measured from twelve different angles "leads" and is recorded over a period of time usually ten seconds. In this way, the overall magnitude and direction of the heart's electrical depolarization is captured at each moment throughout the cardiac cycle. There are three main components to an ECG: the P wave , which represents the depolarization of the atria; the QRS complex , which represents the depolarization of the ventricles; and the T wave , which represents the repolarization of the ventricles.

This module aims to provide nursing, medical, and allied health professionals with a detailed understanding of the electrocardiogram ECG and its analysis and interpretation in clinical contexts in the care of adult patients. The module has been developed to enable practitioners to gain knowledge of the theoretical principles underpinning the ECG to enable a deeper level of understanding of clinically important abnormalities that will enhance the care of the acutely ill cardiovascular patient. The module facilitates the development of clinical skills in cardiovascular assessment and ECG interpretation in clinical practice, as well as the ability to appraise and critique current evidence. The practitioner will also have the opportunity to develop skills in reflecting on their own practice. The content of the module reflects the priorities for care identified by the North of England Cardiovascular Network and NICE guidelines for recognition and response to acute illness in adults in hospital.

How to record a 12-lead electrocardiogram

This article provides a step-wise, practical approach to recording a lead electrocardiogram ECG and explores the evidence base that supports the use of this important assessment tool in clinical practice. A lead ECG is frequently used in a variety of clinical settings, including emergency care, preoperative and post-operative assessment, and primary care. It is used to assess and diagnose patients with suspected arrhythmias, hypertension, coronary heart disease or heart failure.
